How many implants do you need? Does every missing tooth need one?

In short

Not every missing tooth needs its own implant. A single tooth is usually replaced with one implant and a crown, several neighbouring teeth with a bridge on fewer implants. For fixed teeth in a toothless jaw, four to six implants are usual; in the lower jaw, a denture can clip onto two implants. Your bone and the planned teeth decide.

Does every missing tooth need its own implant?

No. The number of implants depends less on the number of missing teeth than on the teeth that will be fitted on them. Cleveland Clinic describes three basic forms: one implant with a crown for a single tooth, a bridge on implants for several teeth, and a restoration of the whole jaw on a few implants.

Mericske-Stern and Worni (2014), who reviewed 30 years of studies on fixed teeth in jaws with no teeth, note that since the 1990s it has been shown that it is not necessary to place as many implants as possible. The table summarises the figures from the sources:

Situation Usual solution Number of implants in the sources
One tooth missing crown on an implant 1 (Cleveland Clinic)
Two neighbouring teeth missing two crowns, or one implant with a cantilever 1–2 (ITI Blog)
Three or more neighbouring teeth missing bridge on implants 2 or more (Cleveland Clinic)
Jaw with no teeth, fixed teeth fixed bridge for the whole jaw usually 4–6 (Cleveland Clinic); at least 4 (ITI, 2023)
Lower jaw with no teeth, denture removable denture on implants 2 recommended, 1 possible (ITI, 2023)
Upper jaw with no teeth, denture palate-free denture on implants 4–6 (Sadowsky and Zitzmann, 2016)

How many implants for a single missing tooth?

Usually one. According to Cleveland Clinic, this restoration consists of one implant carrying a single crown. The neighbouring teeth are left untouched, and the crown is cleaned like a tooth of its own. The comparison with a conventional bridge, for which the neighbouring teeth are ground down, is in our guide Dental implant or bridge.

How many implants for several neighbouring missing teeth?

If several neighbouring teeth are missing, not every one needs an implant; a bridge on fewer implants can close the gap. According to Cleveland Clinic, in a bridge on implants two or more implants carry a bridge that replaces three or more teeth. With three missing teeth, for example, implants are placed at the two ends and the middle tooth is suspended between them as part of the bridge.

If two teeth are missing, one implant with a cantilever is also possible. A 2023 article on the ITI Blog summarises the studies:

  • The cantilever barely affected implant survival; in one study it was 95.7 per cent after five years compared with 96.9 per cent for single crowns.
  • The restorations themselves lasted less often with cantilever bridges (88.9 compared with 96.3 per cent).
  • The most common problems were technical: chipped porcelain, loosened screws, fractured abutments.
  • In the front of the mouth, one implant with a cantilever instead of two implants can help preserve the gum papilla between the teeth.

In the author’s view, cantilevers are a reliable option but call for attention to technical problems.

How many implants for fixed teeth in a jaw with no teeth?

If all the teeth of a jaw are missing, four to six implants are usually placed for a fixed bridge. Cleveland Clinic gives four to six as usual; in the review by Mericske-Stern and Worni, four to six implants per jaw also stand out, with a somewhat wider range in the upper jaw. The 2023 ITI consensus report calls for at least four implants for a fixed bridge.

The differences between four and six implants, the research and the role of tilted implants are explained in our guide Four or six implants.

Are two implants enough for a lower denture?

For many patients, yes. The very title of the 2002 McGill consensus statement sums up the position of the experts involved: the lower denture on two implants should be the first choice for patients with no teeth. A review in Dental Update reports that under this statement, the conventional denture is no longer regarded as the first-choice treatment for the lower jaw with no teeth. The 2009 York consensus statement, published in the British Dental Journal, confirms this view.

The 2023 ITI consensus report summarises the patient perspective:

  1. A lower denture on a single implant is possible.
  2. Two implants are recommended for higher satisfaction.
  3. More than two implants have not been shown to improve satisfaction with a removable denture.

Smith (2005) notes a limitation in his review of the McGill volume: most of the data come from people who continued to wear a conventional full denture in the upper jaw. In the upper jaw, a denture on implants usually needs more implants. The options are compared in our guide Dentures or implants and on our pages implant-supported dentures and dentures.

Which factors decide the number of implants?

The number follows not from a single criterion but from several together:

  • Planned teeth: A fixed bridge for the whole jaw needs at least four implants; a lower denture can be planned with two (ITI, 2023).
  • Bone volume and anatomy: The height and width of the bone and the distance to the sinus and nerve canal are measured on the CBCT scan; if there is too little bone, either grafting is needed or the position and number of implants change. The 2023 ITI report recommends assessing the available bone and the medical history before implant placement.
  • Upper or lower jaw: According to Mericske-Stern and Worni, the number varies more in the upper jaw.
  • Implant size: In the cantilever studies cited on the ITI Blog, all the implants that were lost had a narrow diameter (3.3 mm); what size the bone allows influences the plan.
  • Appearance and gums: In the front of the mouth, preserving the gum papilla can argue for fewer implants.
  • Cleaning and maintenance: The teeth must be easy to clean underneath and to maintain later.

Are more implants always better?

No. Mericske-Stern and Worni report similar results for fixed teeth with different numbers of implants; in the studies reviewed, average survival rates after five or more years were 90 per cent and above. The 2023 ITI report likewise shows no gain in satisfaction from more than two implants under a removable lower denture.

Too few is not sensible either: fewer than four implants are not recommended for a fixed bridge in a jaw with no teeth. The right number is the one that carries the teeth securely and suits your bone.

What does the number of implants mean for your trip?

The number of implants mainly changes the length of the surgery and the number of fittings, not so much the number of trips: implants always have to heal before the final teeth are fitted, so two stays are usually needed. If bone grafting is required, a further trip may be added. How the stays are structured is in our guide How many days do I need in Türkiye for dental implants?; which items appear in the plan is explained in our guide What determines dental implant costs?.

For a preliminary assessment, send photos and any existing X-rays on WhatsApp to +90 532 363 49 54; during working hours we reply in English within 15 minutes. Transfer from Istanbul’s airports and accommodation in Sapanca are included in the treatment fee. How do we decide the number of implants at Rönesans?

We decide the number of implants after an examination and CBCT scan at our clinic in Serdivan. If there is not enough bone, bone grafting and sinus lifts can be carried out at the clinic. In jaws with no teeth, we fit fixed bridges on four or six implants; whether a denture on implants or another solution suits you is assessed at the examination.

Our dentists discuss your case together, and you are treated by a dentist you know from the start. How many implants will be placed, which treatment is planned for which tooth and what it costs are set out in a written treatment and cost plan before treatment begins. Frequently asked questions

Can I find out how many implants I need before I travel?

Not with certainty. The number of missing teeth gives an idea, but the number of implants depends on the height and width of the bone, the position of the sinus and nerve canal, and the planned teeth. That only becomes clear at the examination and CBCT scan. Photos and existing X-rays sent on WhatsApp allow a preliminary assessment, no more.

How does the number of implants affect the cost?

It is one of the main cost factors, but not the only one. Bone grafting or a sinus lift, the type and material of the teeth, and temporary teeth also change the total. A bridge on fewer implants is not automatically the better solution. The cost follows from the written treatment and cost plan after the examination.

Can a denture on two implants be changed to fixed teeth later?

In some cases yes, but additional implants are usually needed. According to the 2023 ITI consensus report, two implants are recommended for a lower denture, while a fixed bridge needs at least four. Whether more implants can be placed depends on the bone and is checked with a CBCT scan. Raise this possibility at the first planning stage.

Is a denture on two implants worthwhile in later life?

Often yes. The York consensus statement notes that the benefits of a lower denture on two implants continue into later life, and points out that a conventional lower denture becomes harder to wear as the bone shrinks. The decision depends on your general health and bone; more in our guide on the age limit for dental implants.
